Admissions Specialist
Cassia, a nonprofit senior living organization, is seeking an Admissions Specialist to support strong partnerships between hospital systems and our TCU and LTC communities across Minnesota. This is a great opportunity for an experienced professional to bring their expertise to a mission-driven organization dedicated to serving older adults.
As our Admissions Specialist, you will help individuals transition from acute hospital care to skilled nursing, rehabilitation, or long-term care. Each day, you will work with hospital partners, families, and Cassia communities to assess care needs, coordinate admissions, and ensure a smooth transition of care. To be successful, you should have a strong understanding of senior living and healthcare services, excellent communication skills, and the ability to build relationships. Licensed Social Workers, LPNs, RNs, and other transitional care professionals are encouraged to apply, but licensure is not required.
Position Type: Full-Time, benefits eligible position is a Home Office position located in Edina, Minnesota, this position is primarily a remote work from home position. Minimal travel is required, as well as some in person field work. M-F with some rotating weekend and afternoon coverage required.
Wage Range: $75,000 - $93,000 Annually depending on experience

Admissions Specialist Responsibilities:
- Communicate and Coordinate Services for New Admissions to Skilled Care.
- Monitor occupancy performance and support census growth through effective referral and admissions management.
- Serve as the primary liaison between hospital discharge planners, family, and incoming residents while new residents transitioning into Cassia’s services.
- Ensure adherence to admissions protocols, processes, and standards.
- The Ability to make admission decisions which include but are not limited to admissions acceptance and denials.
- Complete full skilled care admissions services coordinating services and care throughout a resident’s transition from hospital to skilled care communities.
- Ensure that admissions are completed thoroughly and safely.
- Develop and maintain data tracking and analytics as required.
- Communicate key insights and admission/transitional information to hospital staff, Cassia Site Staff, Families, and incoming Residents.
- Identify opportunities for process improvement to enhance operational efficiency and collaboration.
- Work collectively and collaboratively with team members.
- Licensed Social Workers, LPNs, RNs, and other transitional care professionals are encouraged to apply, but licensure is not required.
