RN Home Health Clinical Manager
Triniti Home Health & Hospice is hiring an experienced hospice Registered Nurse (RN) in the role of a RN Home Health Clinical Manager. Your skills, compassion, and commitment will be valued in our supportive, mission-driven organization.
The ideal RN Home Health Clinical Manager is a clinically skilled and compassionate leader who ensures patients receive exceptional, coordinated care across the continuum. In this role, you will provide both direct patient care and leadership oversight to designated home health personnel within your assigned territory. You will be responsible for delivering high-quality, patient-centered services while ensuring care is well-coordinated, compliant, and aligned with best practices.
Position Type: Full-time, benefits eligible position
Wage Range: $41 - $50 / hour depending on experience
Location: 1111 Highway 25, Buffalo
RN Home Health Clinical Manager Responsibilities:
- Lead and oversee home health patient care to ensure high-quality, coordinated services
- Manage and support nursing staff, including scheduling, coverage, and performance
- Coordinate referrals, admissions, and care transitions with intake and care teams
- Develop and maintain individualized care plans based on patient needs
- Provide direct patient care, including assessments, symptom management, and education
- Communicate clinical updates with physicians to support care decisions
- Ensure accurate documentation and compliance with regulations (OASIS, care plans, orders)
- Manage EMR workflows and resolve documentation issues to support billing
- Monitor caseloads, productivity, and staffing to optimize operations
- Lead team meetings, care conferences, and interdisciplinary collaboration
- Coach, train, and evaluate staff, including hiring and performance management
- Support quality improvement (QAPI) and process improvement initiatives
- Assist with budgeting, reporting, and operational oversight
- Build relationships with community partners to support referrals and growth
- Provide leadership coverage as needed, including weekends/holidays
- Graduate of accredited school of nursing
- Current state of Minnesota RN licensure
- 5 years recent nursing experience in acute, long-term care or home care/hospice/public health
- Home Health Care experience required
- Management experience preferred
- OASIS cert of experience preferred
- Patient care planning and patient teaching skills
- Critical thinker with excellent written and verbal communication skills
- Current Minnesota driver’s license, insurance and daily access to reliable vehicle
