Brookside Senior Living, a Cassia community, is hiring a Service Coordinator to join our team!  At Brookside Senior Living, you’ll work in a supportive, team-oriented environment where your skills are valued, and your contributions make a meaningful difference for our residents.  This is a great opportunity to grow in a healthcare setting. 

In this role, you will ensure our residents are linked to supportive services that they need to continue to live independently and comfortably. You will coordinate and assist with case management services with a focus on wellness and independent living.
 

Position Type: Part-Time, 54 hours per pay period working a varying schedule

Wage Range: $20 - $26 /hour depending on experience

Location: 804 Benson Rd, Montevideo, MN 56265

Service Coordinator Responsibilities:

  • Refer and link residents to services in the community including transportation, legal advocacy, nursing services, and wellness screenings.
  • Assist residents with tasks such as completing forms or contacting resources.
  • Maintain positive working relationship with providers who serve the residents.
  • Work with resident council as an adviser, including attending meetings, taking minutes, and assisting as needed between meetings.
  • Develop and maintain a strength-based, solution-focused approach when working with residents, coworkers, public, and other professionals.
  • Coordinate educational opportunities for residents.
  • Perform additional tasks as needed.

Service Coordinator Qualifications:

  • Bachelor's degree or appropriate work experience
  • Knowledge of and training in elderly and disability services
  • Knowledge of referral processes
  • Demonstrated working knowledge of local services for the elderly and people with disabilities
  • 2-3 years of social service delivery experience. It is preferable for service coordinators to have direct experience working with elderly people or people with disabilities
