Harmony Gardens is hiring a Full-Time Quality Manager (RN) to join our team! This role involves developing, implementing, and maintaining quality improvement programs, and fostering a culture of continuous improvement within the skilled nursing facility. Our ideal candidate will have knowledge of MDS and MDS process as you will be serving as a backup to our MDS Coordinator.
This position works Monday through Friday, 40 hours a week.
Pay Range: $35.52 to $42.84 / hour depending on experience.
Our new campus opened in December 2022 and is only 2 miles away from Maplewood Mall, beautiful parks (Hazelwood and Harvest Park), and a variety of dining options. Our new state-of-the-art senior living community includes independent living, assisted living, memory care, skilled nursing, and long-term care options.
Key Responsibilities:
- Quality Improvement:
- Develop and implement comprehensive quality improvement programs.
- Monitor and analyze quality indicators (examples: MN risk adjusted quality indicators, Casper Report, internal measures) and outcomes.
- Collaborate with interdisciplinary teams to identify, refine and promote and audit best practices.
- Data Management and Reporting (QAPI):
- Collect, analyze, and report quality data to internal and external stakeholders.
- Use organizational specific tools such as My Analytic in Matrix/equivalent in PCC, to drive quality improvement.
- Staff Training and Development:
- Educate and train staff on quality improvement processes, regulatory standards, and best practices.
- Resident and Family Engagement (Annual QOL survey process and result management):
- Engage with Residents and their families to gather feedback and address concerns.
- Implement Resident - centered care practices to improve Resident satisfaction.
- Leadership and Collaboration:
- Lead quality improvement committees and workgroups.
- Collaborate with leadership and staff to develop and implement quality improvement initiatives.
- Serve as a resource and mentor for staff on quality and regulatory matters.
- Empira Program Sustainment
- Responsible for auditing and PDSA (Plan-Do-Study-Act) of sustainment plans for communities that have participated in PIPP (Performance-based incentive payment programs) with Empira.
- For communities who have not participated in previous Empira programs, but wish to implement past learning this position partners with Empira to coordinate strategies, determine and address specific needs, develop tailored action plans, and facilitate the effective integration of program best practices.
- Other responsibilities as determined appropriate by organization
- Examples: oversight of restorative programs, champion for movement, MDS back up.
Skills and Requirements: - Must be in good standing with the Minnesota Board of Nursing and have a current Registered Nursing license.
- Ideal candidate will have knowledge of MDS and MDS process as you will be serving as a backup to our MDS Coordinator.
- Knowledge of quality improvement
- Ability to develop Quality Improvement Plans.
- Knowledge of quality indicators, collection and reporting of quality data.
