Social Worker/Social Services Designee
Haven Homes, a senior care campus located in Maple Plain, is seeking a Full-Time Social Worker/Social Services Designee to support or residents and families. In this role, you will advocate for our residents in our Long Term Care unit, working closely with them and their families to ensure they receive high quality care and services tailored to their needs.

We moved into our premier 149-unit independent living, assisted living, TCU and long-term care senior health & living campus in December 2020. We are located a mere 12 miles from Ridgedale Mall, just off Hwy 12 in Maple Plain.
This a Full-Time, benefit eligible position, working 32 hours a week.
Pay: $26.00 - $30.000 / hour, depending on experience
Social Worker/Social Services Designee responsibilities:
- Provide valuable social services to residents and their families in medical, emotional, physical, or psychological concerns.
- Assist the team in coordinating social services for the facility, individualized plans for the residents medical equipment, counseling or other follow-up treatment plans.
- Commitment to compassion, excellence and quality care.
- Act as liaison between families, staff, and various agencies.
- Act as an advocate ensuring basic human and resident rights.
- Conduct and coordinate Admission, provide appropriate referrals and discharge planning of residents.
- Provides resources and referrals to staff, residents, and family members.
- Additional tasks as required.
Social Worker/Social Services Designee skills and requirements:
- Must have strong social services background.
- Must have knowledge of state and federal guidelines related to long term care and resident rights.
- Long term care experience preferred.
- Successful candidates will be mission-minded with excellent communication skills.
- Licensed Social Worker preferred; however, candidates without a degree will be considered for a Social Worker Designee role.
